6 month supervised diet United health care community

In CT, United does not cover WLS at all....probably need to call your carrier directly to see what your plan covers :)

are you sure it is not just your plan that doesn't cover it at all? I find it hard to believe that some of the really big companies in CT don't cover it.

I have United health care in Illinois, and they had basically no requirements other than severe obesity. They just left it up to my surgeon. They didn't even want to review the claim before my surgery!

I have had two different UHC policies with different requirements.  Through my husband old job,  they required 6 months supervised diet.  

My husband switched jobs last October and the new policy is also UHC. This one only required that I show a diagnosis of morbid obesity for 5 years.  I had my PCP print out my annual visit for the past 5 years.  I was approved quickly.

Same here - I have UHC through my employer.  It took 2 days to get approval from the surgeon's office.  It depends on which plan you have.

I have United Healthcare Community Plan(medicaid) in NY.  It did cover WLS.  The requirements were six months supervised diet, pysch eval, and BMI >40 or BMI>35 with comorbidities.  Of course your surgeon may have other requirements.  Here is a policy I found
