Cards for when you eat out

Cleveland Clinic gives them out. You should ask your surgeon/center if they have something like this.
However, I have never used them. First off, most items on the kid's menu are not appropriate after surgery -- they are usually fried and fatty things like grilled cheese, breaded chicken tenders, etc. Secondly, the restaurant is under no obligation to honor it. Finally, there are usually plenty of great options that don't require me to announce to a total stranger my medical information.

They're usually provided by your surgeon, if you get one at all.
I have one, but I've never used it. I just use the line, "I'm not super hungry, can I just order off the kids' menu/order a half portion?" I'm always polite about it and I've never had any trouble.

First of all - nobody is under any obligation to honor those cards being whipped out, so ask yourself if you really want to divulge your personal medical information to strangers?
Next - the other poster is right that you can simply ask for a half portion of a healthier entree than a childs menu crap-food. I simply say I have a small appetite but hate leftovers. More are happy to oblige than not.

Keep a few things in mind: Having this card in no way entitled you to order off the kids' menu. Restaurants are under no obligation to honor them, and why should they be? Anyone can print one off. Most restaurants who do honor them will also honor your polite request, with or without a card. And please, PLEASE remember that the cost of the actual food is the smallest part of what it cots a restaurant to serve you a meal. Please tip your server not on the price you pay for your 'kid's meal', but for the service he or shew gives you.
I personally would be very embarrassed to use one of these cards. I never offered to pay more when I could eat the legs off the table, so I'd feel really bad about asking to pay less now that I don't.
I have a card, but have never used it either. I eat out frequently for work functions and personal travel. I had RNY 4 months ago and have lost 70 pounds since then. I find that most menus serve shrimp, which has worked very well for me. I usually just ask if they have a side of grilled shrimp or scallops or something like that and every single restaurant has been extremely accommodating. It usually comes out plated very beautifully and is typically very inexpensive. Also, remember to tell them that you want little to no oil. We are on vacation in Hilton Head Island right now and I have split meals with my family. I have ordered items from the appetizer menu and split with my family. Today I ordered a small bibb salad (vs. the entree) because I finally get to eat salad again (who thought you could miss that?) and asked if they could give me a few shrimp, which they did no problem. I am not eating beef yet, so seafood is a great choice and they will typically prepare fish in a variety of ways that will work for you post-op. Just ask for what you want and you should have no problem getting it.
