Have you selected a surgeon for your surgery? - their office staff includes people who can help navigate the insurance approval process.  That maybe a good resource to begin with.

Good luck,

Man insurance companies offer case management. Call the insurance and ask. They can also give you all the specifics of what you need to do. Nobody here will be able to help you since every policy is specific to what the employer set up. I am a case worker so I know how you feel. I wish you luck.

On this website at the top in the blue line across the type click on the RESOURCES-drop down to Medical Necessity- It will show you a letter you can use to send to your insurance company when trying to get pre-approval. It would be best to research doctors, select the top ones you would consider, go to their seminars and talk with their representatives and she how helpful the are in helping secure insurance approval. You need a great surgeon with an awesome support team for you. Research and start asking others and you will find someone that has had the surgery or knows someone who has. Networking is great to gain good info. Good luck.
