Surgery or Not!
I ate like a lumberjack-that's how I got to 300+ pounds. I alsobought into the food pyramid and ate lots of grains and tried to avoid fat.
Now I eat protein first, some veggies, and full fat dairy.
You may be different, but most of us either were clueless how much we consumed or outright lied to ourselves about how much we ate.
I fight badgers with spoons.

One thing I would suggest is that you give weigh****chers or some other weight loss plan a try and eat three meals everyday of what you are allowed and nothing else for a week and see what happens.
None of the bariatric surgeries are cures and most people gain much weight back unless they eat less fruit, 1 per day, add more protein and cut back on starches, the white foods. Even some vegetables are very starchy. So about the only unlimited foods are things like lettuce, broccoli, green beans.
Before having any surgery, I would continue to do research and start back eating 3 small meals and a couple of small snacks each day plus 64 oz of water. Include 60 or more grams of protein. The liquid protein drinks also make me sick. It is the artificial sweetener which I cannot tolerate and I haven't found any without the sweetener already added. Giving up carbonated beverages is pretty much required, but you will not eat 2 T per meal forever. Just a few days and that will be all you want at that time.
I was part of a bariatric surgery group for several years. Two people had lap-band, none had sleeve because it wasn't being paid for by insurance since then it is. The rest of RNY. Out of ten people, several had problems with just not being able to eat and kept losing weight for a long time. I have lost track of the person with the worst problem with this. The two lap bands lost about 40 pounds and then stopped, and started gaining back the weight they lost. it is not easy to eat solid protein with a lap band. With huge amounts of chewing, it still gets stuck sometimes. You can't eat much at one time as with the sleeve, but you can graze which adds pounds quickly. Those who had RNY lost the most weight initially, but all of them have gained a good deal back because as you said, the body does adjust to not eating much and then you need even less. No one of the ten has gained all of their weight back
Since you take cortisal medications or produce too much, it is true weight loss is difficult for you. Try a weight loss plan, and follow it first for several weeks.
