So, my insurance company covers the bariatric surgery itself with a provider they determined but not the preoperative appointments or followup care that is required to get the surgery from that specific provider. Is anyone else confused by this?

If you're confused, call back and get more information; there's so much difference between insurance plans that nobody here can tell you what they meant.

Each case will be different.  Depending on the severity of the plagiocephaly, and when it began, the insurance company may accept some cases, and deny others (sometimes of the same “cir****tance” and “type”).  Not many insurance companies will automatically accept your claim.  In order to successfully appeal, you need to objectively collect information that will help prove your case.
