I know,just such a simple thing.

I took a typed piece of paper with my surgeon's name, address, phone and fax numbers typed on it. I basically walked in for my annual pap smear and said, "Hey look... before you start talking to me about how I need to work on my weight and stuff... we need to talk. I have been really concerned with my weight for years. It is very important to me to avoid any of the usual co-morbidities... so what are your thoughts on weight loss surgery? I know it is only a tool and I am still going to have to work my tail off, but I think I am ready to get this show on the road. How about you? Can you write me a referral letter? Here's the information."

He was all about it. I had my letter by the end of the day.
