To determine the best procedure for you - you really need to look at your own body and how it reacts to dieting attempts in the past.  You can research and determine what you need better than any doc can in a 15 min consult.  Read all you can about the RNY, Sleeve and Duodenal Switch (because it's highly recommend for BMIs over 50).  Google each, and look to the boards here for the forums specific to each procedure.  You want to see what they are asking about, complaining about and raving about.  Ask some questions of your own.  To to the WLS Revisions and WLS Regrets pages to learn the worst-case scenarios.

Take what you learn and apply it to what you already know about yourself.  Ignore the subjects of convenience and inherant fear.  Not all docs do all procedures, but traveling to get what you want may be worthwhile.  Call your insurance to see what your options are there, too.  They may limit your options.
