Even if they did it is individiualized to what your employer wants to include. So if you are looking for insurance information I would suggest calling the number on the back of the card and finding out what you want to know. Then it will not be anecdotal information.

Thanks so much. I wanted to called but I don't want to tip them off and not be approved. VSG is covered I just wanted to see if there are hard to get approved.

When I was looking to get VSG I called the customer service line. They emailed me a very nice detailed list of what I had to do. It made life a lot easier. When people don't get approved it usually because it is not covered due to the employer excluding it or they did not do all the requirements. If you meet all of them they can't just not approve you.
