FWIW, some surgeons don't use a pre-op diet because 1) they don't believe that significant changes can occur with only a 2-week diet, and 2) they would prefer that patients go into surgery without the physical stress of a liquid diet.
I didn't have one because my RNY was planned to be open.

My surgery was laparoscopic and I had no pre op diet at all. Normal right up to three hours before surgery for liquids and ten p.m. The night before for solids. I did ask my surgeon why he doesn't require a pre op diet and he said everything that Lora said and also that he knows his way around a fatty liver in the event that there is one. Mine turned out to be not fatty at all according to his surgical report. This is not the usual approach though. Most do have the pre op diet requirement.
Yes, I had fatty liver prior to surgery and because my BMI was so high, my surgeon required four weeks of a pre-op diet. I had read stories of people who had not followed the pre-op diet and the surgeon closed them up because their liver had not decreased. It made me paranoid that it could happen to me so I started my pre-op diet six weeks prior to surgery.
The first few days are the worst, but the motivation to be ready for surgery was strong. It's completely doable.
