To find my medical records

I had surgery at the bariatric treatment center in Belvidere Ellen Neu on November 19, 2001. I found out that the treatment center is no longer in business. I am trying to find anyone who had surgery there and knows who I can contact to obtain my medical records.

Did you have your surgery at the center or a hospital?  If a hospital maybe they will have your records or at least now where they are stored.

I had my surgery at the center. There is a hospital where the center was located. I left a message for their records department. I hope they have or know where my records are located.

you could also look online for the state licensing board for either the physicians or facility and they may be able to point you in the right direction. good luck!

My Dr. Left the clinic when it was open so I don't know who to look for. My Dr. Eric Schlesinger is located in Tempe, AZ. THANKS!

You could be out of luck.  Depending on state law, the treatment center may have destroyed records that were over 5-7 years old.  In Nebraska we have to keep them for 7 years but can destroy them after that.

I hope that is not the case because my local bariatric said they could not see me without the records. I just want an internal checkup since its been 13 years since my surgery and may want to look into a revision.

If it was 13 years ago, it's unlikely the records still exist.  Most places destroy them after 10 years.

Have you explained to them that it's been 13 years and those records no longer exist? I'd suggest calling around to find a more reasonable place.
