I just selected HealthSpan (formerly Kaiser) for 2016. I did have Medical Mutual, which does not cover wls. These were both through Healthcare Marketplace (Obamacare)! HealthSpan does have bariatric coverage, but require a 6 month pre surgery program. Hope this helps.
