Upset w/my pcp regarding letter of necessity HELP!!!!! Shabani Institute

My doctor gave me a form for my pcp to fill out.  Ask your surgeon if they have a form. If not, ask the pcp, kindly put it on letterhead. My insurance company needed it, maybe Medicare does too. Good luck!

Not sure if you already got this issue resolved - but....I would suggest speaking to the Office Manager of your PCP's office and let them know you must have it (if you find out it is a requirement). Sounds like a PCP that I would not want to go back to if they can't take a few minutes out to type a letter for you and sign it. Just my thought. I've never experienced that with my PCP before. They are always willing to write/type letters when needed.
