I am in Alabama. How does it work with a revision with insurance?

Start by calling your insurance plan and asking them. You can also go online to the insurance website and look for information.

Each carrier and plan has different requirements.

AS Kate said, it will depend on your individual insurance plan.  Some cover revisions and some do not, and the required criteria for approval if it IS covered will vary a lot from one policy to another.
