Navel under artificially created navel hole

Several years ago I had a panniculectomy surgery and an artificial belly button was created that sits atop my natural belly button.  Like a cavity real belly botton on the bottom and artificial belly button hole on the surface of my skin that makes my stomach look natural.  Problem the opening was always small and I had to clean the skin and real navel by inserting q-tips inside to swab my navel and keep it clean.  Now the artificial opening is so small and tight I can hardly get a q-tip in there to clean my navel.  No surgeon wants to touch me and they keep sending me all around.  What can I do?
