pre-op anyone else have IEHP/Medi-cal go through appeals?

I have IEHP through medi-cal and I went through the 6 month program with a nutritionist and my primary care sent a referral to see a surgeon and it was denied. Has anyone been through IEHP 's appeal process? Any tips?
