Molina Medicaid Approval
I just wanted to let people know that Molina of Ohio, was really quick at approving my weight loss surgery. As long as the information is there it should take around 7 days to be approved. The test that were needed was
H-Pylori
H-Pylori must be negative
Toxicity which is a drug & alcohol test
Aids test
Cardiac Clearance stating you are cleared by cardiologist for surgery
Pulmonary function test stating you are cleared for surgery
6 months of supervised weightloss classes
Co-morbifity high bloodpressure, diabetes, arthritis, sleep apnea, BMO higher than 35, or some other type of weight related health conditions.
Nutrional class which will count as one of your 6 month weight loss classes.
Physcological clearance for Bariatric surgery
Any weight history you have for the last 2years if available
Sleep study test within the last year
