Did they give you a hard time with approval? I'm curious to know because I called them to make sure I had all the info/criteria I needed and they wouldn't answer any of my questions :/ should I just talk with my doctor?

Your doctor almost certainly is not familiar with all the insurance requirement of all the different policies out there.  I would call back and ask to speak to a supervisor.  They should give you all the info you need about your policy.

Please note: I AM NOT A DOCTOR.  If you want medical advice, talk to your doctor.  Whatever I post, there is probably some surgeon or other health care provider somewhere that disagrees with me.  If you want to know what your surgeon thinks, then ask him or her.    Check out my blog.

I Googled Excellus SimplyBlue Obesity and found the guidelines. Now, that is if treatment of obesity is a covered benefit of your specific policy, as in if your employer excludes.
