I had the DS November 1, 2012

My insurance required a 12 month Doctor Supervised diet.  I think that's ridiculous, but I won't get into the debate.  However, I had already been under doctor's care for weight issues and blood pressure for over 3 years consistently.  All my doctor had to do was submit 12 months worth of consecutive notes (1 note for each month) showing my BMI, weight, diet plan, medication/supplements, and exactly what I was doing to attempt to lose the weight.  My weight loss attempt wasn't successful, but this may have been what helped me to get approved so fast.

BTW - my first approval was through BCBS - TX.  Then my employer switched Insurance providers and I used the same paperwork to get approved through United Health Care.  Approval took less than 2 weeks both times.

Another tip I dug up all those Weigh****chers books - you know the ones where you get a stamp and weight written in along with my Diet Center records --- they weren't sequential but they showed that this was not a fly by night decision. I was actually surprised with the amount of time and energy I had put into weight loss and the number of years I had been at it. These records coupled with doctors visits seemed to do it. My dieting history started @ 11 yrs old with the help of Dexedrine - wow!
