Need some assistance with my appeal...please!!

Stay on her about this - and I would also recommend calling insurance *again* to verify *again* and ask them to call her to set her straight.  It's okay to attack this from two angles.

 Did you do weigh****chers, Jenny Craig or any of the other programs in those last 3 years.  From what my surgeon explained to me Even if you didnt do them for a total of 3 years each but you did, lets say,  Jenny craig for 6 months,  South Beach Diet for 5 months, and Weigh****chers for 1 year You should add that all into your appeal.  My surgeon said that most times they dont even ask for documentation.

You might ask your insurance carrier for something in writing on their policy specifically for your plan on what their requirements for bariatric surgery is. Then you have an idea of what you really need, not just what someone is telling you. Whenever I would call and ask my insurance company I would get a different story each time, so I went online and found out exactly what my plan covered. It makes it easier if you  have to appeal too. I am still in my appeals. Hang in there! Good luck! :)

Thank you all so much for your replies. I just wanted something to present to the insurance company showing how their 3 year requirement is not the norm. The longest requirement I found was 12 months. The average is 6 months. Keep praying for me. When I checked the other day my request for pre-auth was "pending". Have a great day!
