psych eval clearance letter...

Does anyone have a copy of their psychiatric evaluation clearance letter for weight loss surgery? The "dr" Im using doesnt have a template and is "contacting his associates" for one .. so thought if I could get a template (just remove your name / private info from it first) - I could STOP THIS "hurry up and wait game". PLEASE - if you have one.. could you PLEASE email it to me ... [email protected]

Thanks all for the replies - however I dont need a list of items the psych needs to "ask" / put in letter - as I have already given him one of these - he is currently searching for an "approved template" to work with (basically something he can just input the info into regarding me specifically - that has already been proven to be accepted by insurance - as he is afraid - being his first clearance - of me not getting approval solely due to his letter)
But I called his office today and they have informed me that he has in fact gotten this template that he was looking for and they should have my letter ready within a week. (guess it takes a week to type one page document.. they need to take KEYBOARD at middle school level to help them with this problem lol)
But again - thanks all for the replies - will keep updates of all things in my blogs :-)

Being that you're not the only patient that they have, it can take some time for professionals to get the availability to write your letter/report. They do have other things that they have to do; patients to see, notes to complete. Mental health professionals are grossly overworked and underpaid. Give them some break. Many professionals give a 2 to 3week timeframe for any document/note/letter. Often, professionals write these during their own time, especially if they work in a clinic or group practice, they might not even be getting paid for writing the letter.
