Where you have your surgery

Mine did, in fact they told me to start using it three days before surgery.  (Don't think they swabbed my nostrils though unless they did that when I was knocked out.)

I chose a doctor who had great success at a nearby hospital so if anything went wrong he could fix it.  I severed my relationship with his office during the first year but he is and always will be my wl surgeon and I will always be his patient and when I am in need he consults on all my testing to make sure I will continue to have a positive result for as long as he practices.  I had a blocked bile duct last year and during my ER trip to figure out what was going on my wlsurgeon was called and all the tests he requires to check my rny were performed to try to figure out the source of my severe pain.  I can't imagine you can reach a doctor in Mexico for any sort of follow up in that same situation.  Personally I would never leave the US to have surgery ever unless it was an emergency and I had no choice.
