procedure costs....

Hello guys, im starting the 3 month weight lioss requirement by my insurance and was wondering about the cost and how the insurance normally charges. I have aetna and they said they would cover 90% after a $400 copay, also my doctor has a program fee of $500 and the nutritionist is charging me $200. now my question is, after the surgery does the insurance bill me the other 10% or do i have to pay up front before surgery???? thanks for your help guys
