Insurance denied revision due to low BMI

I did some math last night.  Here are the Stated facts:
Current: 32 BMI
Surgery: 35 BMI
8 pounds gain necessary to get to 35 BMI and surgery.

This means she will be at 29 BMI if she LOSES 8 pounds.  Another 10ish get's her to 25 BMI ('normal').

18ish pounds or a life threatening surgery.

We each have our own issues, and I don't know this persons.  I, for the life of me, can't imagine how 18ish pounds could dramatically affect quality of life. Again, I'm not her.  Certainly, though, many formerly obese people have unrealistic self-image, and it is something at this point that should be looked at VERY carefully.

What kind of medical necessity drives you to want to undergo a DANGEROUS revision operation, besides what appears to be about 20-25 lbs off of being a normal weight?
