I have Aetna and was approved after:

1-6months diet history (within the last 12 months)
2-saw a psych and nutritionist
3-showed that i was struggling with weight at least 5 years back

My BMI was over 40 and I had sleep apnea, diabetes and high blood pressure.

Hope that helps!

I have Aetna and was approved in February 2010 with the following:
1. BMI of 42
2. Weight history for the past 2 years showing my BMI was over 35 (because I have a co-morbidity)
3. Sleep study showing I have Sleep Apnea
4. Doctor's notes showing I have Hypertension and various drugs did not work to stabalize my BP
5. Psych evaluation and clearance
6. 3 visits with NUT (not totally 3 months, just 3 visits a month apart) I had an appointment Nov 17, Dec 16 and January 16. That met their requirement. I had heard on the boards that you in fact need 4 NUT appointments because the visits need to span 3 full months. In my situation, this was not the case. They were just looking for 3 visits.
7. PCP support of surgery letter.

I was approved after my first submission in about 10 business days.

My husband was also approved after his first submission, but he got his approval in 24 hours.

From what you write, it looks like you have a good shot, b/c you have sleep apnea, Aetna requires your BMI be 35 or higher, so you should be good with a 39. Keep us posted on your progress!

I was approved after:

6 month supervised diet w/my PCP
- NUT evaluation
- Psych evaluation
- 2 year medical history
- hypertension was my co-morbid condition
- a BMI of 34.7 the day of my initial consultation.  By the time I concluded my 6 months, my   thyroid was failing and my BMI was higher by the time I was approved, but that was the BMI  my surgeon submitted.

I see no reason why you would be denied unless your plan has a bariatric exclusion.

Best wishes
