FMLA questions
Eligible employees get 12 weeks per year of FMLA for a serious health condition for themselves or a family member. Yes, you can apply for intermittent FMLA and use it a day at a time. You can be approved for up to 6 months at a time and need to reapply after 6 months.
The employee and doctor need to complete the forms. Their supv signs off on the form the employee completes and then the employee sends everything to HR. Your supv should NEVER see the medical reason due to HIPAA. Once it's approved a letter will be sent to your supv letting them know the dates the FMLA will cover and the type of leave (LOA vs intermittent).

Hello,
I use FMLA one day at a time, but it really depends on what your case worker approves you for. I have psoriatic arthritis and some of the medication makes me very sick, so I am able to use two days per week up to 37.5(this is my normal work week).
Your doctor would need to fill out a FMLA form which can be found in HR or if your company has a intranet site, you may be able to find it there.
