It seems you should measure your bmi in the morning without clothes, but of course at the doctor's office you are clothed.  How do you measure it?

What's the purpose?

If you are being weighed to qualify for WLS, you want it to be as high as possible.  Hence, you curve over so you measure a shorter height, you have clothes on, wear your heaviest shoes (Dr. Martens, anyone?), and fill your pockets with coins.

If you are being weight to note your progress, you want it low.  So, naked as the day you were born and after taking your height measurement standing straight.

I am exaggerating, of course.

My cardiologist's office subtracts 2 lbs for clothes and shoes.  Every other dr's office  I've ever been to uses clothed in stocking feet.  I do the same to track my weight except for the time I got on the scale outside the supermarket hoding a grocery bag and wondering how I managed to put on 10 lbs for the month.

I shoudn't worry! Unless you are right on the borderline between two grades, it won't alter the figure. My nut always allows 3lbs for clothes in the winter and 2 in the summer!

BMI is only a very rough guide to health anyway- it makes no allowance for muscles, bone size etc. All your football athletes would be severely obese by BMI as would our rugby players!

Highest 290, Banded - 248   Lowest 139 (too thin!). Comfort zone 155-165.

Happily banded since May 2006.  Regain of 28lbs 2013-14.  ALL GONE!

But some has returned! Up to 175, argh! Off we go again,
