Does anyone have Empire BC/BS PPO and ............

We are switching from BC Cali HMO  to Empire BC EPO and everyone including my sugeon says ita  great move and it will be easier to get an approval from Empire. Sounds like they are one of the few insuance companies that "get it" when it come to WLS

I had to have a signed paper from my doctor saying I was on a supervised diet for a year.  My doctor signed that.  Also, the psych test, stress test, echo cardiogram.  Any reports from the doctor.  That's about it.  If you call SHPS, they will tell you what you need.   Patti
