How To Get Med Records If Can't Remember Doctor 5 Yrs Ago?

Hi everybody, I have Blue Cross Blue Shield and they require a 5 yr history of obesity (which I have been obese for much longer than 5 yrs!), but they require medical records. We moved out here to the suburbs 3 and a half years ago, and I can't remember my primary care doc from our former house! I'm wracking my brain and I just can't. Does anybody know of a different way to get medical records even if you don't remember the doc?? Thanks! Barbara

If you have the same employer providing the insurance, they may have the records.  And, if you have the same insurance, they may have the info as well.

You insurance company will be able to tell you who your doctor was.

Unfortunately, our insurance company has changed several times. I don't remember who our insurance company was, either!

just a tip Barbara BC/BS (some anyways) require a 6 month supervised diet with a nutritionist .. so you may want to book those appointments now if you already haven't .. just trying to help you get a jump on it ..

I had this happen to me before also.  My doctor had prescribed medicine for me.  I went to my pharmacy and with everything being computerized, they were able to give me a print-out of all my mediciations and lo and behold, there was the doctor's name.

Likely when you went to the new doc, he/she asked you your previous dr's name.  It is probably in your new doc's records. Very good idea about the pharmacy!

Do you remember the name of the clinic he was at? They could look it up for you. I'm a receptionist at a clinic and use Google a lot when patients don't know the address, doctor's name or clinic name. If I have either the doctor's name or clinic name I can find it. I have even found it with just the address and town the clinic is in. My insurance didn't require a nut it just had to be supervised by a medical person.
