Ok, I have surgery on 2-2-09 and I have had to leave work for drs appointments and labs etc a couple weeks before my surgery.

Will they go back and prodate things so that it covers the days missed for pre op appts etc? Also how many days do you usually have to submit info to the office about the claim before its auto rejected.

Im so worried about this and I dont want to lose my job, we have really strict job restrictions about days missed

Please contact your HR Benefits office asap.  They are the experts and can interpret policy for you so you won't get any attendance problems.  I work in HR and sooo many times people won't call or come in to see me until its too late and then I can't do anything to help them.  Forms cannot be backdated.  FMLA starts when you go in and apply for it.  Very important thing to remember in all of this!  Call them today or drop by to see them and get the ball rolling.  As long as you follow policy and turn in all the required paperwork from your doctor and etc., you should have no problems.  Also check to see if you're covered by short term disability.  This will pay you a portion of your salary while you're off work.  Best of luck to you!
