That is an insurance that we have never dealt with but generally the best approach is to call the insurance company and ask them if bariatric surgery is a covered benefit. If it is then ask them what the specific requirements are for the qualification. Most of the time they will tell you as long as it is determined medically necessary. They will rarely tell you their definition of medical necessity. Ask them to tell you what the diet history requirement is for medical necessity - most insurances now require 6 months of physician supervised dieting with monthly visits documented in the physicians office notes. Also remember to always get the name of the person at the insurance company that you speak with, document every conversation that you have with them.
