The insurance companies hold all the cards, since they are paying the bills. I don't know that mine would have accepted Weigh****chers or any other diet organization as acceptable. I had to go to the dietitian in my physician's clinic each month, and her notes had to be submitted to the insurance. I tried appealing the rejection, but lost the appeals.
Call the insurance company to find out what they will accept as proof of dieting attempts. Start working with your physician on a supervised diet. The insurance may require seeing progress notes from an office visits devoted specifically to a discussion of your weight loss, what calorie level you're following, food journals, exercise journals. Those were all things that I had to submit to finally get the authorization.

This journey is a process. For some it may seem it happens over night, but for others it can be long and tenuous. It appears you've been chosen for the long/tenuous route. There are lessons to learn, perhaps include patience and fortitude.
You know, you could go to WW and ask for a copy of the weekly card, if you haven't kept your own books. Otherwise, talk with the doctor who would follow you for a year, on your dietary program and keep plugging along. Many insurances will accept WW's, but there has to be discussion with the doc as well. So, set something up. Perhaps the surgeon's office has a program that might work better for you than WW. These things are all so individual. If you can kick some of those poor habits pre-op, it will make life post-op so much easier.

I was also denied by my ins because I didn't have the supervised diet. When I called my ins co and was asking them what would be required, they informed me that they didn't accept Weigh****chers or Curves. I find that stupid. I am trying to figure out what to do now. If I had only known this seven months ago when I started everything, I could have done it. Now I have my goal set and they want me to wait another 6-12 months. I don't know if I can handle that or not.
