Help finding a new PCP in Southeastern WI!

I have decided if I want to have surgery, I'm going to need to get a new PCP. It's been over a month and she still hasn't written a letter to send to the insurance. I don't know what to do. I wrote a letter and gave it to her and she said she wanted to dictate one herself. That's okay with me, but I didn't plan on waiting over a month for a letter. Does anyone know of a very WLS friendly PCP in SE WI that takes United? Probably I'll wind up having to go to Racine or Kenosha, which is okay. My current PCP is in Kenosha. I just want to be able to have the surgery done, sooner rather than later!

Hi: Don't know if you found a pcp yet, but my dr. is very well versed with wls and is very nice. His name is Dr. Michael Dawson and is out of West Allis Memorial Hospital.
