hello every one , i have a ?

i was taking phen phen in 1997 , the dr office that provided this to me was called primary care physician , in waukesha. they have since closed down , i need my records from them , i'm thinking who ever bought them out might have my records. the problem that i have is that i can't find any info on the office , and i can't remember how to get there , can anyone help? i really need the record for my appeal .

i would think if ud just call the city they might be able to help u out with finding out who out bought them and such... ~Nikki~
