Denied follow up care by Humana Insurance

I had my surgery 8/27/03 by Dr Kumar.Humana will not authorize any more follow up visits because they said Dr. Kumar is not in my plan. Why did they approve my surgery? No other Dr. will see me for follow up care. What should I do?
