Medical Necessity Letter

I LOVE the sample letters provided on this site, but the link for the letter for Medical Necessity from Physician isn't there! I am so disappointed, especially since my PCP admitted to me that he doesn't know much about WLS. He is more than willing to support the procedure, but since he isn't very knowledgable on the subject matter, I worry that his letter won't be as good as it will need to be. Is anyone willing to share the letter that their PCP or surgeon wrote so that I might use it as a guide for my doctor's letter? I would REALLY APPRECIATE it! Send to, [email protected].
