Wisconsin Electrical Employers Health & Welfare Plan Insurance

Has anyone had to deal with this insurance?  They cover gastric bypass - but the list to qualify is one whole page.  I've called the insurace company to get items clarified but get the run around.  Example " The eligible individual has demonstrated to the reasonalbe satisfaction of the Trustee or their delegated representatives his/her inablility to control weight over a period of five consecutive years."
