You guys have been so very helpful. I already have my appeal letter done to the insurance company and I contacted them to find out how to do the appeal since the letter did not indicate it, only gave me a number to call. They are sending me the form and procedures how to do the appeal. I have obtained all my documention from the PCP, urologist, pyschologist, gynecoligst for documentation supporting each one of my medical conditions. I have also contacted an attorney *****presents claims that are denied for obesity, and he is willing to help out also if this claim is denied a second time. Hopefully it will not have to come to that but I need all the backup and help I can get. Thanks again for all your replies.

FYI - depression is most definitely a life-threatening condition.  Suicide is one of the most common causes of death in people below a certain age.  Depression causes suicidality, and obesity makes depression worse (and harder to treat). Oooohh, it makes my blood boil when people don't treat mental illnesses as real diseases. (Make sure that your Axis IV diagnosis is "Major Depression."  If it isn't, give your doctor a reason to change it - talk to him more thoroughly about it.)
