Midwest Security insurance
Yep I do. Had surgery a year ago. Just make sure what your policy says. I didn't have any issues with mine. However, I did keep calling so find out the status. Once I had the letter of approval I called the surgeons office. However, when you find out you were approved check to see how long they give you to have surgery. They gave me only 90 days causing the surgeons office to scramble. Good luck and keep me informed.

I have Midwest Security. I believe it was up to the company I work for to choose a specific insurance plan to offer its employees through Midwest Sec. Lucky for me, the WLS was covered through my companies plan - with certain criteria. Such as you had to be 100 lbs. overweight, and bmi had to be over 40 - which, was the same criteria for my clinic to accept me as a lap-band candidate. All in all, when I had problems, the customer service was very helpful - just have to keep on top of it and keep calling them with issues. Also, even though my surgery was "pre-appoved" my anesthesiologist was not in the plan - I didn't find that out until later and had to pay outside my PPO. So check on that too, before you go into surgery. Most of the time, the anesthesiologist isn't assigned until right before the surgery, and you don't have a choice in the matter.
