PLEASE HELP question concerning MEDICAID

I am on Medicaid. I meet all the criteria for the surgery. My doctor wrote a letter for me saying that because of my medical issues the surgery would be right for me. My Question is after my doctor agrees and I meet all of their criteria what do I do? Does my doctor send it in, or does he refer me to the UW? Can I use my own doctor as a diet doctor? I have called the number on the back of the medical coupon, and they are of no help. What about the exercise program? do you have to join a gym? hope not as money is not there. Thanks for any infor you can give.

My friend told me he had to have 6 months of doctor suprvised (spelling?) diet.then he went and found a doctor I beleive one of those at UW and then that doctor sent everything in..I would ask him who the doctor was but he is in Iraq working.. My idea about what you need to do is make some calls find the doctors who will deal with medicaid and them do some talking with the doctors to see who you want to do it..I'll give you the person I talked to at Virgina Mason her name is Myrene Bruce 206.223.6636 that is a virgina mason number in seattle she could tell you if any of the doctors there will take medicaid.I had my WLS done there with Dr.Moonka.
