diet history

does anyone know what is expected? Im sure a detailed diet history is just a list of diets how long I was on them and how much i lost right? Also i need to include a personal letter, a bit about myself. what do they want to know? Im thinking of trying to give a visual for ex: I am 4ft 9. Laegally a dwarf. (under 4 10) I have a list of dallas cowboys and of all 42 of them only 11 weigh more than me. So to recap I am the height of an average 10 year old, and weigh more than a pro foot-ball player. Are these the things they want to hear? do you know of any profiles that have sample letters? Elsa

Hi Elsa, My profile has the letter that I sent to my insurance company. It was a detailed diet history and personal letter about the problems I had because of obesity. I hope it helps you prepare yours, because I had a hard time coming up with what I was going to write. It took a good 3 or 4 hours to prepare. Good Luck and I hope your able to get approval. Craig
