You will probably get a call from Kaye telling you if your insurance most likely will cover it and/or if they have a 6 month diet or whatever. If they will cover or if you are self-pay, you can make an appointment. My insurance originally required a 12 month diet, so they told me not to come in yet. If you don't hear from them in the next couple of (business days), you should call them.

I would tend to agree with Karin; if you dont manage your own goals you can get lost in the Shuffle. Kay is working less than part time so you might smart to call your insurance company to find out what your requirements are and see if you can start getting everything compiled. Everyone tends to "do their job" but no one ever goes beyond the call of duty... just my opinion... all the girls are nice but sometimes nice doesnt cut it.
Had I not done alot of leg work and daily phone calls to Cigna I would not be scheduled for surgery later this month.

Self pay should call, if insurance you should find out what your insurance requires and if they even cover it. The office told me 6 mo diet (pcp and dietician visits) but it was actually 12 months (verififed by insurance and later the office). Once you are within 4 months, call and start your testing (consult is first)
