Ah, so you're looking for someone to repair a hernia and bill insurance for that, and then pay the difference out of pocket to add in VSG plus the extra operating room fees, etc.
I don't know what you've tried so far, but I would recommend getting a list of bariatric surgeons in your area and cross reference that with a list of general surgeons from your insurance company. Then start calling and explaining that you want hernia repair and VSG, but insurance doesn't cover the VSG so you'd like to self-pay the difference.
The bariatric surgeons might have extra fees for their "program" and that's something to be aware of.
You also might find that it's easier to go out of country.

I was self pay for my surgery and chose to travel to FL. I had a very good experience with Dr. Shillingford in Boca Raton. The cost was exactly what they said it would be and he repaired my hiatal hernia at the same time. He did not bill anything to my insurance.
