Every insurance company is different. I was under the BMI but had high blood pressure and type two diabetes. I also found out I had sleep apnea during my pre-op testing. I was told that not only did I have to lose weight and reach my pre-op goal but I had to maintain it. In other words, I had to weigh the same or less at each dr visit before the surgery. It's stressful but doable.
