Sliding back toward being diabetic- suggestions?

Just got my labs back, and although I slighltly improved the A1c and fasting sugar level, I am still solidly in the prediabetic stage. This was where I was at this time last year as well.  I was diabetic prior to surgury, although newly diagnosed about a month prior so no intervention transpired. I also had gestantional diabeties and a family history. How many out there are experiencing a similar slide? I feel pretty strongly that my insulin resistance had returned, as I can no longer quickly drop a few pounds with my typical diet adjustments. If you did slide, were you able to reverse and how? I do know that the guidelines indicate to lose 10-15 pounds, exercise will help combat the insulin resitance, and watch the carb load carefully. Exercise is out due to ongoing issues from a double mastectomy, and I get I need to work harder at cleaning up my diet. Anything else?

Unfortunately - eating more carbs - even complex carbs - can stress the system.  What works for me is Keto style diet, more fat, less carbs...moderate proteins...

The carbs in the signature are complex carbs. During my weight loss phase I struggled with low blood sugar, so carbs were needed. Fast forward to now, and I do need less carbs... however still can't do the under 40 grams thing. I try to stick to green leafy veggies and fruit primarily, but have been wrestling with sugar and working to banish that.

I have been dealing with a lot of low sugars...unfortunately fruits can cause that for me. Or complex carbs.  

I know my body is very efficient in processing almost any carbs, even some fiber, into sugars... I learned that the hard way. Any carbs - even complex make me crave more carbs. 

I treat fruits as "high sugar snacks" and I allow them in my diet once in a while... And only some of them...and only with combination with fats and some proteins... 

It took me a while to discover that... 30 lbs to discover what works what what does not... 30 lbs regain and a lot of tears and frustration... And more angry tears... 

There were days that my fingers were swolen and bloody from poking them to measure my BS every 15 min to discover how my body reacts to different foods.. Day after day... I had to take weekly breaks to allow my fingers to heal... 

My endo was amazed with my detailed data... 

I have insulin resistance, my body makes too much insulin,  and I have to be on steroids, and that causes higher fasting blood sugar... At the same time I was able to lose 25 of the 30 lbs of regain... The last 5... I am up and down... 

But I know carbs - complex or simple are not my friend. I still eat them from nuts, non starchy veggies, and... some from fruits (when I am tired being very strict or when my BS crashes because I did not take my meds on time). 

This is the only way I can control my BS and my weight.. 

BTW during my honeymoon time - for app 2 years post op - I could eat complex carbs with no problem... I had hard time keeping my weight up in year 2. And I ate carbs.... Crackers, whole grain bread and wraps, fruits and starchy veggies... 

But after year 2 - my body got smarter and learned to convert all carbs into sugars and run them into body fat... Or the carbs caused very wide BS fluctuation.. 

I am 7+ years post up...Maintaining most of my weight loss.. (size 6-10) with very little exercise (bad back, busy work) ... It is not easy... But it can be done...

Thank you HALA-  my body has ajusted as well. No more low blood sugar issues like early out. I also spent a couple of weeks last summer constantly checking sugar levels to determine exactly what happend when I ate (fill in the blank). Learned a lot from that for sure, and I believe the adjustments I made after that helped. I just need to keep tweaking until I can get those numbers down.

I also noticed that acid - makes me crave for more sugary -starchy stuff..(I am back on omeprazole)

Some artificail sweeteners (or combination of stuff in diet drinks) makes me go crazy for sugars -carbs.

Plus - when I eat the carbs makes a huge difference  what and how much I ate... (now I reserve more carbs for late afternoon - evening)

Good luck in figuring out how to adjust the diet..

I think for sure acid is an issue. I have been dealing with some stomach issues, and the gastro doc put me on 40 mg of omeprazole twice a day, then bumped it down to once per day. There is an absolute difference and now I am a little better at identifying when it is an acid issue, and for sure some of the grazing was triggered by this. And no, no hard feelings at all! Carbs + diabeties = trouble if not managed.

My fasting level has risen over the past two years. However, I have had a chunk of regain and was eating poorly in between my 3 healthy meals. Basically, too much sugar and I wasn't really watching my carb intake.

I got it down by completely ditching the sugar. The only sugar I'm eating comes in the form of lactose and a fresh fruits that are low on the GI. While I am not anti carb at all, I do think the quality of carbs is very important and if you are truly insulin resistant you need to keep those carbs low, even if they are "healthy" carbs.

I know your history with carbs, so I'm going to urge you to reduce gradually to a level that works for you. I think if you can get below 60 carbs per day (15 carbs per meal) you will be doing fantastically. This may be very hard for you. I seem to recall you eating about 30 per meal during weight loss. I'd give it a try for a few weeks (maybe 4) and have the doc retest your fasting glucose and see what happens. It's not that long of a commitment and if it's not working you can look at another method.

I am going to try to drop down a little more, and get it under where I was in weight loss to the greatest extent possible. I am similar in that my carbs do come primarily from dairy and green leafy veggies. I have even dropped most of the fruit I was eating, not that it was too much anyway and items low on the glycemic index as well.

 I have a NUT appointment and follow-up with the doc on the 22nd. I am also looking into a more "alkaline" diet. I have no idea if it is legit, but inflammation is what is causing all of the grief with the scarring around my implant and having to redo the surgery again. If I can reduce inflammation maybe it will help in that arena as well. I will see if they can offer some options as well.

And let there be no doubt that part of the issue was my eating sugar. I work on that daily with some days being better than others. Ugh.

If you don't mind my asking, what was your latest A1c and fasting sugar level?
