If you want to get fancy, or you do much in the way of exercise, I'd suggest checking into something like the RoadID. I have one that I wear when cycling, and it lets me keep all of my medical information up-to-date online. It has a phone number, serial, and PIN that an EMT or doctor can use to call and get everything they might need to know-- emergency contact information, names and phone numbers for all of my doctors (including bariatric surgeon), current meds, medical history, the whole deal.
