sleeve or rny

Actually, the sleeve is drastically different from the lapband.  Unlike the lapband, the sleeve adjusts the metabolism due to changing hormone production via removal of the stomach.  It's nowhere near as powerful in some respects as the DS or the RNY, but ghrelin production drastically falters.  In fact, a part of the metabolic boost in the DS is due to the sleeve - the malabsorption certainly accentuates it, though.  In the case of the VSG by itself, some peer reviewed report ghrelin levels remain altered months after surgery.  There's definitely a metabolic boost with it.  It's not as profound as the DS or RNY in some respects, but even with those the body acclimates a little to the malaborptive procedures, too.  I've spoken to people who gained a ton of weight on the DS even, or lost very slow, due to their food choices.

As for 2-3 pounds a month, that sounds more like "user error" than the surgery.  The patients in the support groups I visited average 5-20 pounds a month for quite some time (6-12 months out).  Most of the studies I've read show the sleeve has comparable results to the RNY long-term, and perhaps slightly less.  The DS remains the most effective over time, unsurprisingly, and will likely be for a while.

I follow a ketogenic diet post-op. I also have a diagnosis of binge eating disorder. Feel free to ask me about either!

Agree that there is a little extra with the sleeve, but it is officially still considered to be a restriction-only procedure.  If the OP didn't get but only marginal results with the band, it is foolish to expect any significant difference with another restrictive procedure.
