You must always take a quick look at the person and find their weak spot then point it out.

Examples, Yeah but I did not have ears that stick out, a big nose like that, all that belly fat, ugly shoes, just find something about them to make fun of and believe me it will hurt them

Sadly I also believe this is the ONLY thing people like this can understand. If it were a male saying something like this, I would have said something about his small penis.
