insurance required diet
Mine ended up being about five months . The dietician I met with gave me a prescribed Diet and exercise plan to follow . I recorded everything in my fitness pal and printed the reports and took them to my visits.
I replaced breakfast with a protein shake and stuck to a low calorie low fat diet.

Every insurance is different. I called mine as well as thorought read about WLS requirements on my insurances website for more detailed information. I have cigna. Mine required a 3 month medically supervised weight loss plan whi*****luded monthly documention from a doctor or dietician including weight, diet, and exercise. I seen my primary MD as well as the dietician on my surgical team monthly and all my info was documented. My primary MD wrote a letter for me indicating the dates I seen her with all the required info. All my paperwork was submitted to my insurance on 7/20/15 and I got the call yesterday that my insurance approved. My surgery is tentatively set for 9/3/15. I tried this whole process over 3 years ago with my old insurance aetna and was denied. So I followed everything to a t this time and even did more and went beyond. I want the surgery so very bad. Check with your insurance, but our 3 month medically supervised had to be medically supervised, meaning our doctor had a form he needed to fill out every month and that was submitted with everything else to get insurance approval. We didn't mind it all actually, the eating plan was very similar to what we do post-op, just smaller quantities now, it was good practice before surgery.
