For me it wasn't a big deal.  My leak test was scheduled for the day following surgery.  I wasn't thirsty, and definitely not hungry.  My lips were dry and a very mushy lip balm made it all better.  I was also given cold lemon flavored swabs after surgery.

Many surgeons want to make sure that there isn't a gap or defect in your sutures, so they don't want anything in your stomach that could flow into your gut until the leak test is performed.

I just had my surgery 3 days ago and I was NPO for 24 hours. My Surgeon is very thorough and I am following his orders to a T. He knows what he's doing and if he says NPO then it's for the best. My leak test was done while I was in surgery so I know that has nothing to do with nothing to drink for 24 hours.   Good luck to you on your surgery.
