How do I change my weight on my little chart and how do I list my surgery date?

As you update your weight in Health Tracker, it will automatically update your little chart :-).  If you want a little blurb under your chart, you can add that in "my introduction".  Under "My OH" there is a place for "medical history" and you can add your surgery date there.  Hope this makes sense.  I went through a lot of opening and closing menus to find all the features!  Good luck!
